  • click on the orange “Get a card” button
  • type in your name in the spaces provided
  • type in your address in the spaces provided
  • type in your contact number in the spaces provided
  • type in a valid email address in the space provided
  • type in your annual net income amount in the space provided
  • type in how many years you have been at your current address
  • type in your date of birth in the spaces provided
  • type in your social security number in the spaces provided
  • type in your mother maiden name in the space provided
  • choose your preference on how your statements will be delivered
  • OPTIONAL: choose whether or not you want to ADD card security for an extra fee
  • review the terms and conditions and then click the box to agree to the terms and conditions

  • confirm your information and then view the results
